Seasonal Waiver Agreement
Below is a copy of the seasonal waiver you are agreeing to when registering your player with the Louisville Soccer Club.
I understand that it is my responsibility to provide my child(ren) with proper clothing and equipment for practices and games to participate in the Louisville Soccer Club. Such clothing and equipment include the Louisville Soccer Club jerseys, shin guards, socks that cover the shin guards, and soccer cleats. Players who do not come to practices/games with proper cleats and shin guards will not be allowed to participate for health and safety reasons. The Louisville Soccer Club community league does not have any rules regarding the color of socks, shin guards, or soccer cleats.
- Tips on Cleats
- The Louisville Soccer Club requires players to wear soccer cleats for safety reasons. The most important reason is to prevent players from slipping on the field and causing injury. Football and baseball cleats are NOT acceptable. We also recommend that before every practice or game, you double-tie your soccer player’s cleats. Many practices and games have had to be stopped for a player to tie their shoes. If gone unnoticed, this too can be a safety issue.
Guidelines for Jewelry
- In general, soccer rules are becoming stricter with regard to jewelry. The Louisville Soccer Club is enforcing the policy to not allow any jewelry during soccer matches. The only exception is players who have recently had their ears pierced and cannot remove them. In this case, they will be asked to cover up the studs with a Band-Aid.

- I understand that at this time we cannot honor requests for players to be on specific teams or placed on the same team as another player (except for siblings in the same age group) due to the large number of players we have. Players are placed on teams by a randomized computer system.
- I understand that practice day(s)/times will be determined based on the volunteer coach's availability. If you need a specific requirement for your child(ren), we encourage you to volunteer to coach.
- I also understand it is my responsibility to get my child(ren) to all games and practices on time. Missing practices and/or games not only causes them to miss out on learning new skills but also hurts the other players when it comes to games.
- I agree to abide by the LSC Zero Tolerance Policy on Referee Abuse.
- I also agree to abide by the LSC 24-hour rule.
As the parent or legal guardian of the child named above, I hereby give my full consent and approval for my child to participate as a team member in the Louisville Soccer Club. I understand that there are certain risks of injury in the practice and play of soccer as well as in traveling and other activities incidental to my child’s participation and I am willing to assume these risks on behalf of my child. I hereby certify that my child is fully capable of participating in soccer and that my child is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in these activities, except as listed above. In addition to giving my full consent for my child’s participation, I hereby waive, release, and hold harmless the Louisville Soccer Club, its officers, coaches, sponsors, supervisors, and representatives for any injury that may be suffered by my child in the normal course of participation in soccer and the activities incidental thereto, whether the result of negligence or any other cause.
